What is telehealth?
Telehealth enables you to have a consultation with your health care provider via video call or telephone. This means you don't need to take time off work to travel to an appointment. Consultations are delivered via Halaxy, a secure video telehealth platform. It's very easy to use — you'll receive a link to click at the time of your appointment. No downloads required. You just need a computer, tablet, or mobile phone with a microphone and ideally a camera. If you don't have access to these, a phone call consultation can be arranged.

Medicare rebates
Medicare supports the use of telehealth, and you may be eligible for a rebate for consultations with a Credentialled Diabetes Educator. Ask your GP about a Chronic Disease Management (CDM) plan or Team Care Arrangement, which can provide rebates for up to five allied health visits per calendar year.

What does a Credentialled Diabetes Educator do?
A Credentialled Diabetes Educator (CDE) supports you day-to-day with blood glucose monitoring, medications and insulin, meal planning, using devices like CGMs and insulin pumps, and making lifestyle changes that fit your life. A CDE also works alongside your GP and specialists as part of your care team.
How does a telehealth diabetes consultation work?
Appointments are held over video call using the Halaxy platform. You don't need to travel, just a device with a camera, microphone, and internet connection. Bookings and reminders are handled through the same system.
Do I need a GP referral to book an appointment?
No, you can book directly. However, a GP referral under a Chronic Disease Management (CDM) plan allows eligible patients to claim a Medicare rebate on their visits.
What conditions and situations can you help with?
Support is available for type 1 diabetes, type 2 diabetes, gestational diabetes, pre-diabetes, insulin pump and CGM setup and troubleshooting, pre-pregnancy diabetes planning, and general chronic health condition management.
What does a consultation cost, and are rebates available?
