Services

All individual and group services are provided via telehealth, so you can access expert diabetes support from the comfort of your own home — no travel required.

What is telehealth?

Telehealth enables you to have a consultation with your health care provider via video call or telephone. This means you don't need to take time off work to travel to an appointment. Consultations are delivered via Halaxy, a secure video telehealth platform. It's very easy to use — you'll receive a link to click at the time of your appointment. No downloads required. You just need a computer, tablet, or mobile phone with a microphone and ideally a camera. If you don't have access to these, a phone call consultation can be arranged.

Services offered

  • Type 1 diabetes
  • Type 2 diabetes
  • Diabetes in pregnancy — pre-pregnancy planning
  • Gestational diabetes (GDM)
  • Pre-diabetes and diabetes prevention
  • Insulin pump support and commencements
  • Continuous glucose monitoring (CGM and Flash) — commencements and ongoing support

Medicare rebates

Medicare supports the use of telehealth, and you may be eligible for a rebate for consultations with a Credentialled Diabetes Educator. Ask your GP about a Chronic Disease Management (CDM) plan or Team Care Arrangement, which can provide rebates for up to five allied health visits per calendar year.
