Accessing Diabetes Education
You can book an appointment with Angie directly, with or without a referral.
Referrals are also welcome from GPs, endocrinologists, specialists, nurses, dietitians and other health professionals.
If you have a valid GP Chronic Disease Management (CDM) referral, you may be eligible for a Medicare rebate for your consultation.

Please note: Medicare rebates are only available with an eligible GP Chronic Disease Management referral.
