Funding Options

Services can be funded in a number of ways:

  • Facility-funded — billed directly to the aged care facility
  • Home Care Package (HCP) — residents with a Home Care Package may be able to use their funding for diabetes education
  • NDIS — NDIS participants can access diabetes education under their plan (subject to plan approval)
  • Medicare — individual residents with a GP-referred Chronic Disease Management Plan may be eligible for a Medicare rebate
